Southeast Warren families will now
have a simplified method to register students into the Southeast
Warren Community School District. Parents are now able to register
online to review and update existing information, and register new
students into the district. This will eliminate the need for much
of the paperwork. Although the enrollment process occurs online,
there may be a few forms and items that the district will need in

ONLINE STUDENT REGISTRATION
Here is an introductory video about the online registration process:
Log in through the Parent Portal and complete the online registration
information. If you don’t have a Parent Portal login, contact:

- Confirm existing student enrollments (select this first if
you have a currently enrolled student)
- Add additional students for 2017-18
- Student name should be exactly as shown on birth certificate
- Provide health and medication information

students new to SE Warren who do not have a sibling currently enrolled,

Student - Parent Questionnaire
Complete the Parent Questionnaire/Release Form. This is a Google
Forms document that will be electronically submitted to the school.
It will include:
- Lunch charging preference
- Military connected status
- School volunteer availability
- Various release and authorization statements
- Handbook acknowledgement
Parents of new students and prsechoolers
should complete both forms in Step 2.

The Chromebook Agreement should be completed for any student receiving
one for the first time (7th graders & new students in grades
7-12). If a previous agreement has been completed, it is not necessary
to do another one, however reviewing the information is encouraged.

If you wish to apply for free or reduced meals and school fees,
complete the application above and submit it to the district office
(see below). If you received a notice that your children are automatically
eligible through state direct certification, you will need to sign
the fee waiver/hawk-i release form that you received with your notice
and return it to the district office.

Log into the online payments page school website and pay all appropriate
fees. Payment can also be check or cash, and mailed or delivered
to the district office in Liberty Center. You will be notified of
any outstanding fees from last year, and these should be paid at
this time using the Miscellaneous category.
Fees per Grade Level
If you have applied for free or reduced meals, you should wait
to get the notice of the application results before paying any school
fees, as they may be waived or reduced. However some fees are not
eligible to be waived/reduced and still should be paid.
Optional: You may also put some money in your child’s lunch
account and purchase activity passes.
Students in grades 7-12 only
Documents needed for sports participation should be submitted to
the Jr-Sr High school office prior to the student’s participation
in any school-sponsored sport, including cheerleading, dance, trap
team and rugby. You can also request these SE Warren forms if visiting
these Indianola clinics: Mercy, The Iowa Clinic, or Unity Point.

At the Jr-Sr High School, Liberty Center
Parents can get assistance with the registration process, get questions
answered, visit the school nurse, transportation director, dean
of students, secretaries and administrators. Paper copies of all
registration documents will be available. Class schedules will be
available for students in grades 7-12.
Chromebooks will not be issued at this time; they will be distributed
to students the first week of school. The Chromebook Agreement and
deposit should be completed before August 23rd for new students
or students getting them for the first time. Returning students
who have already paid their deposit do not need to pay again, although
you may need to add funds if the deposit has been used for damage
